I had a beautiful 8" pot that went as yours did. I treated like Wendy
recommends below (I use powdered sulfur rather than cinnamon) and saved a
4.5" pot sized plant. In addition, I got a keikei on one of the flower stems
that is now in a 4" pot. The 4.5" is now in a 6" and doing its blooming
thing on 2 stems. I know now to check the medium more frequently. I've seen
a specimen growing on a 3" thick, 12" round slab of tree fern. I'm thinking
hard on how I'm going to treat it next.

If this were my plant,
1... I would unpot & remove the bad stuff.
2...Bath in a Physan solution.
3...Sprinkle cinnamon on any wounds.
4...Repot in a mix of your choice, in a tight fitting pot.
5...Pray a lot!
