Default where to find 15-0-15 fertilizer

Your Co-op would probably carry it or be able to order it for you.

The recommended mix of fertilizer for centipede is 15-0-15. Has anyone
found someplace that stocks it. It does not have to be 15 but the N,
and K need to be the same and the P to be 0. In fact if the N is lower
than the K then I can substitute with ammonium nitrate to make up the
difference. So 10-0-15 or such would be ok.

Pop, look for potassium nitrate for your K source.
You might want to ping Ray Barkelow at firstrays.com to see where he gets
it in bulk for the MSU formulation fertilizer he sells.

Actually, look for greencare MSU formulation fertilizer. 19-4-23

Also, ping the crop-sci folks at NCSU. They made mention of its use
in growing greenhouse tomatoes.
