Default how to improve a yard's soil?

How does the grass survive or do you have to reseed each fall?

If the grass is living ok, then leave it alone.

If you want to plant flowers or other things then mix in as much organic
matter as possible or replace the clay with topsoil. When I say add
organic matter, till it in, add more, till it in, repeat as long as you
can. You can't have too much organic matter as it will break down over
the years.
