Most likely you want the "General Permit" which the USDA site refers to as
"PPQ 622" In order to get this permit you will need to fill out an application for it. If I am not mistaken the application for the General Permit is form "PPQ 621". If the orchid plants in question are on the Appendix I list and intended to be imported into the USA you must also get a CITIES import permit issued by the US Fish and Wildlife Service. Plants on the Appendix II list do not require the Cities Permit from the USA, however they DO require an export or re-export permit or an export certificate of artificial propagation from the country of origin. Usually the seller can provide this for you at a charge but the US Fish and Wildlife Service can probably help you locate the government agency in the foreign country responsible if the seller has no clue. A clue-less seller in the country of origin is probably a sure way to get your plants confiscated at the border. Some of the information in this paragraph came from the USDA "Circular PPQ Q.37-10" which will be send along with your General Permit once you have submitted "PPQ-621" I have always found the staff at the USDA PPQ office in Riverdale to be very helpful when contacted by phone.
