The leaves look chloritic. This suggests the poor thing is being
watered with hard tap water. And/or left to stand in water damaging
some of the roots. Worth checking it hasn't got meally bug under the
leaves sapping the plants energy too.

You must water orchids and other epiphytes with rainwater if they are
to survive for a decent length of time. Also chelated plant food for
orchids (even Miracid would do at a pinch) will help it recover from
the mineral defficiencies with a bit of luck...
