Default Sphag and Bag - When is enough

My general rule-of-thumb is the same as for keikies - it's ready to pot up
when the plant has about 3"-4" of total new root length.

I tried the Spag and Bag from http://www.firstrays.com on 3 dendrobiums
that did not appear to be growing new roots, and were going downhill.

2 died but one is doing quite well. At what point should I repot though?
A picture link is attached. Size can be estimated based onthe standard
plant tag and the Floral wire in the picture

Thanks
