Fill out the forms, mail them in. The key is to list all continents from
which you expect to import (Asia, Australia, Africa, South America and
Europe ought to cover you..)

APHIS will send you some paperwork, including some yellow and green labels
to be sent to the vendors from whom you wish to import.

End of story.

If the vendors are on the up-and-up, the stickers (one each on the outside
and inside of the box) will lead the packages to the nearest APHIS station,
and than on to you at no charge.
