Leaf Zone added once per week

You don't say how much light you have, but if you really have bright lighting
(3 wpg or more), Leaf Zone once a week is not enough. Leaf Zone contains only
iron and potassium. You need a complete fertilizer, like Tropica Mastergrow or
Seachem Flourish. And you'll probably need to add it more often than once a
week.

Even then, it can be hard to keep java fern healthy in a bright tank. It's not
the light, it's the nutrients. It has a hard time competing against
fast-growing stem plants for nutrients.

I keep a separate tank for slow-growers like anubias, java fern, and java moss.
It has moderate light and no CO2, but the plants grow like crazy. While ferns
and mosses never do well in my high-light tank.
