Default in spike, or just a new psudobulb

I have a Bulbophyllum medusa, it's my best orchid --in the sense that
it is never sick and it just keeps growing and growing, it's put out a
whole new leaf in just about two weeks! There are also many other
growths, but I don't know if they are just going to be more leaves...
or if maybe (please please please) it is in spike.

How can I tell?
