Yes, you should have done. I received my seed about two weeks ago. I
seem to remember that the seed form came with the June journal. The list
itself is certainly in the June journal.
The list looks rather like the kind of white piece of paper which just
has your address on for postage normally, but it helpfully has a warning
on it saying something like, 'This is your order form; don't throw
away'! Could you perhaps not have noticed it?

The Cyclamen Society operates its seed distribution in quite a
different fashion from the usual seed exchanges. They publish in
(or in association with) their June journal the list of taxa with
a code letter for each, plus an order form. Since only one genus
is involved, it's feasible to give each conceivable taxon a
separate code in advance, and to take orders in advance. You have
to provide a list of alternates, iirc.

Check your June issue of their journal for further details.
